What does CFU actually mean (and why it's not enough)?
CFU stands for Colony-Forming Units. It's a microbiology measurement: the number of live bacterial cells in a sample capable of forming a visible colony when cultured under standard lab conditions. On a supplement label, CFU is the count of viable bacteria the manufacturer guarantees at a stated point in time — usually either at manufacture or at end of expiry.
The CFU number has two real problems as a buying signal.
The first is the manufacture-vs-expiry question. Many probiotic labels disclose CFU "at time of manufacture" — meaning the actual viable count at the moment you swallow the capsule is lower, sometimes much lower. Bacteria die during storage. Heat, humidity, and time all reduce viable count. A "25 billion CFU" capsule labelled at manufacture might deliver 5–10 billion by the time you take it. A more transparent label states CFU "at end of expiry" or "guaranteed until expiry" — meaning the number on the box is what arrives in your gut.
The second is the species composition question. CFU counts total live cells, not whether those cells are the right cells for what you're trying to support. 50 billion CFU of a generic gut-bacterial blend is not the same supplement as 50 billion CFU of a women's-specific composition that includes vaginal-native species. The total looks identical. The biology is not.

The three things that matter more than CFU count
If CFU isn't the right comparison, what is? Three things — in this order.
1. Are the species named individually? A label that lists "Probiotic Blend 10 billion CFU: Lactobacillus species" tells you nothing about what's actually in the capsule. A label that lists each species separately — Lactobacillus rhamnosus, Lactobacillus reuteri, Lactobacillus crispatus, and so on — is at least transparent. Proprietary blends in probiotics serve the same function as proprietary blends in any other supplement: they let the brand cut costs on the more expensive components without you knowing.
2. Does the composition include vaginal-native species? Most "women's probiotics" on UK shelves are gut probiotics with women's marketing. They contain species that thrive in the digestive tract — useful for gut health — but not the species that dominate healthy vaginal flora. Vaginal-native species (Lactobacillus crispatus, Lactobacillus gasseri, certain Lactobacillus rhamnosus strains) are different. Research suggests these species may colonise the vagina more reliably than gut-native species after oral administration. If the goal is supporting vaginal flora specifically, the species list matters more than the total count.
3. Is there prebiotic support? Probiotics are bacteria. Prebiotics are the fibres that feed bacteria. A probiotic without prebiotic support is sending bacteria into your gut without giving them anything to eat — which limits how well they survive and establish. The clinical research on long-term probiotic effect typically uses formulations with prebiotic support included. Why most "women's probiotics" are actually gut probiotics
This is the part of the women's probiotic category that the marketing doesn't say out loud: most products positioned as "women's probiotics" contain the same species as the general-purpose gut probiotics sitting on the next shelf. The packaging is different. The composition often isn't.
The vaginal microbiome is genuinely different from the gut microbiome. A 2003 randomised placebo-controlled trial by Reid and colleagues first demonstrated that specific Lactobacillus strains — L. rhamnosus GR-1 and L. reuteri RC-14 — could reach and colonise the vaginal tract after oral administration. Subsequent research has extended this picture: oral probiotics influence vaginal flora through at least three pathways, including direct bacterial migration via the perineum, an IgA-mediated immune signalling axis between the gut and vaginal mucosa, and the estrobolome pathway where gut bacteria affect circulating oestrogen levels that feed vaginal Lactobacillus populations.
The implication is straightforward. Not all probiotic species are equally suited to influencing vaginal flora. The species that occur naturally in healthy vaginal tissue — particularly Lactobacillus crispatus, which dominates the most stable vaginal microbiome states — are the species best positioned to support what they evolved to do. A "women's probiotic" that contains only generic Lactobacillus acidophilus and Bifidobacterium species (both gut-native) is fundamentally a gut formulation with a pink label.
The single most useful question to ask of any women's probiotic is: does this formulation contain species that occur naturally in healthy vaginal flora? The answer is on the ingredient list. L. crispatus on the label is the strongest single signal that the brand has formulated for the vaginal microbiome, not just labelled for it.
The UK regulatory blind spot — why every probiotic claim sits in a grey area

What this means for buyers: every health-adjacent claim on a probiotic label exists in a regulatory grey area where the brand is implying something the regulator hasn't endorsed. Some brands stay carefully on the right side of the line (factual ingredient declarations, no functional claims). Some brands sail closer to the edge. None can point to an EFSA-authorised claim and say "this is regulator-approved." Patterns emerge. The market splits broadly into two camps: high-CFU products with broader species lists (Femme Flora, Free Soul) and lower-CFU products with research-anchored strain identifiers (Optibac being the cleanest example).
